- Diabetes Mellitus (DM)
- Cardiovascular Disorders (CVD)
- Chronic Heart Failure (CHF)
- Chronic Lung Disorders (CLD)
- Chronic Kidney Disease (CKD)
Beneficiaries qualifying for a C-SNP must have a verification form completed by the provider treating the condition(s). Once approved, beneficiaries can enjoy additional benefits such as:
- Monthly Over the Counter and Food Allowance (*)
