- Cardiovascular Disorders (CVD)
- Cardiac Arrhythmias
- Congestive or Chronic Heart Failure (CHF)
- Chronic Lung Disorders (CLD)
- Chronic Kidney Disease (CKD)

You can enroll in a Chronic Condition Special Needs Plan (C-SNP) anytime during the year upon diagnosis. This qualifies as a SEP (Special Enrollment Period.) You can join anytime, but once you join, your chance to make changes using this SEP ends. |
Beneficiaries qualifying for a C-SNP must have a verification form completed by the provider treating the condition(s). Once approved, beneficiaries can enjoy additional Special Supplemental Benefits for the Chronically Ill (SSBCI) such as monthly Over the Counter or Food Allowance. (*)
